Primary-Secondary Transition Program in Saint-Léonard and Anjou
Since 2019, the Primary-Secondary Transition program (T2)—a local service offered by the Y—has been actively supporting young people and their parents in Saint-Léonard and Anjou.
The intended impact: fostering young people’s academic success and social success by supporting a smooth transition from elementary to high school.
What is the Primary-Secondary Transition Program?
T2 is a free program that was launched in Pointe-aux-Trembles in 2011, then enhanced in 2019 through Saint-Léonard’s neighbourhood action plan as part of the Collective Impact Project (CIP), in collaboration with Réseau réussite Montréal (RRM), the Centre de services scolaire de la Pointe-de-l’Île (CSSPI), and several other partner organizations.
Today 9 high schools and their catchment schools on the CSSPI territory benefit from the services of T2 agents, supported by 5 community organizations: the YMCAs of Québec, Service d’aide communautaire Anjou (SAC Anjou), Équipe RDP, Coup de pouce jeunesse Montréal-Nord, and CDC de la Pointe.
The YMCA offers T2 in 2 high schools and their 11 catchment schools (10 in Saint-Léonard and 3 in Anjou).
Annually, the Y supports close to 1,800 students through this program.
What Kind of Services Are Offered to Students Transitioning to High School?
T2 agents offer thematic workshops in all Grade 6 and Grade 7 classes on key topics: changes in high school, stress management, myths and realities about high school, values and influences, school organization, etc. They also provide one-on-one support to students who need assistance, who are facing challenges, or who are feeling apprehensive about the transition from elementary to high school.

Our Goals for a Successful Transition to High School
- Reduce students’ and parents’ apprehensions about high school
- Increase students’ knowledge about how high school works and about the school and community resources available to them
- Support the creation of bonds between youth and the adults who are important to them in their community
- Help strength students’ personal, social, and organizational skills
